The Simple Steps of Setting Up Your Plan

Step 1. Provide the necessary information to Oregon Legal Center (OLC) either online from this website or by filling out the forms and mailing/faxing them to OLC. We need you to fill out two forms: a. the Family Information Forms and b. the Asset Information Forms.

a. First, Fill out the Family Information Form:

Click here to use our Secure Server (SSL) to fill out the Family Information Forms on-line to submit your estate planning wishes to OLC. Click here for a list of the information you will need to complete the forms. There is no charge and no obligation when you submit the information.

Or Married Couples click here to download the Family Information Forms For Married Couples in Adobe Acrobat Reader (.pdf) format so that you can print them from your printer, fill them out manually, and then mail/fax the completed forms to our office. Unmarried Individuals click here to download the Family Information Forms For Single Individuals in Adobe Acrobat Reader (.pdf) format so that you can print them from your printer, fill them out manually, and then mail/fax the completed forms to our office.

Or call Oregon Legal Center at 503 635-6235 and speak with either Mr. Noble or Mr. Ellis and they will fill out the Family Information Forms as you speak with one of them on the phone.

b. Then Fill out the Asset Information Form:

Having problems with Acrobat Reader? If the forms don't open in your computer the problem may be that you have an old version of Adobe Acrobat Reader on your computer. Click the Adobe Reader Icon which follows to get the latest version of Adobe Acrobat Reader (Version 7).

Step 2. Meet with Mr. Noble at OLC's office in West Linn to verify your information and to carefully analyze which Plan works best for you. Have all your questions answered. You will be given a quote on the price of your plan.

Step 3. Sign your Plan Documents at OLC's office in West Linn. You pay for your plan when you sign the Plan Documents. Only when you are satisfied that the Estate Plan meets your needs, do you pay for your Plan by check or credit card.
