The Simple Steps of Setting Up Your Plan
Step
1. Provide the necessary information to
Oregon Legal Center (OLC) by filling out the forms and emailing, mailing or faxing them to OLC.

do one of two things: (1) fill it out on your computer and email it back to us,
or (2) print them from your printer, fill them out manually, and then
email/mail/fax the completed forms to our
office.
Unmarried Individuals click here to download the Family

do one of two things: (1) fill it out on your computer and email it back to us,
or (2) print them from your printer, fill them out manually, and then
email/mail/fax the completed forms to our
office.

Having problems with Acrobat Reader? If the forms don't open in your computer the problem may be that you
have an old version of Adobe Acrobat Reader on your computer. Click the Adobe
Reader Icon which follows to get the latest version of Adobe Acrobat Reader
(Version 7).

Step 2.
Once we receive your filled-out form, we will email you a more detailed
form so you can provide the detailed financial information we need to prepare
your Will Plan or Trust Plan.
Step 3.
When we receive your filled-out form with the
detailed financial information, we will call you and schedule an office
appointment.
Step 4.
Sign
your Plan Documents at OLC's office in West Linn. You pay for your plan when you
sign the Plan Documents. Only when you are satisfied that the Estate Plan
meets your needs, do you pay for your Plan by check or credit card.
